dc.description.abstract Nursing is a professional health service that is directed towards the promotion and maintenance of health, treatment and prevention of diseases and the restoration of optimal functioning of the individual, family and communities. To be able to meet the various needs of patients and family, and thus give quality care to them. Nursing care has moved from the physical care of patients to a more complex approach which includes the psychological, spiritual and intellectual needs as well as that of their family as a whole (Nursing process). To achieve ultimate goal, the nursing process requires the cooperation of the family, the patient and the community. Also, education is given to help prevent help them to know their right pertaining to their care. The patient and family care study entails the diseases and to rendering holistic care to the patient and family starting from the day of admission till discharge. Individualized nursing care is the area of interest in the study. The nursing process is a deliberate problem-solving approach for meeting a person’s health care and nursing needs. It consists of a sequence of steps in the following order; assessment, diagnosis, objective/outcome criteria, planning, implementation, and evaluation. Assessment is the systematic collection of data to determine the patient’s health status and identify any actual or potential health problem. Diagnosis is the identification of actual, potential and collaborative patient problems whereas planning is the development of goals and outcomes, as well as a plan of care design to assist the patient in resolving the diagnosed problems and achieving the identified goals and desired outcomes. Implementation is the actualization of the plan of care through nursing interventions and evaluation is determination of the patient’s responses to the nursing interventions and the extent to which the outcomes have been achieved. .
