ItemPatient/Family Care study on Diabetes Mellitus(Holy Family NMTC Library, Berekum, 2022-08-01) Gladys AgyeiwaaNursing has undergone much improvement over the years. It owes much of its body of knowledge to the influence of Florence Nightingale (1820-1910), a woman who pioneered and brought much respect to the profession through her vision. Nursing has basically moved from taking care of the sick to extending the care to include the family members and the community in general through the use of scientific methods and techniques which are employed to help solve problems. Quality assurance is a dynamic process through which nurses assume accountability for quality of care they provide. To ensure quality nursing care within the contemporary health care system, mechanisms for monitoring and evaluating care are under scrutiny. As the level of knowledge increases for a profession, the demand for accountability for its services likewise increases. Individuals within the profession must therefore assume responsibility for their professional actions and answerable to the recipients for their care. The changing scene of nursing care has brought into being, the patient/family care study, as a partial requirement by the Nursing and Midwifery Council of Ghana for the award of Registered General Nursing certificate. It is in this regard that I chose a patient with diabetes mellitus for the care study. The patient/family care study is an academic exercise written by final year students of nursing institutions and involves detail account of comprehensive nursing care rendered to a particular patient within a period of admission through to the day of discharge, there is a follow up to ensure a continuity of care.

ItemA Patient / Family Care Study on Uterine Fibroid(Holy Family NMTC Library, Berekum, 2022-08-01) ATTO YEDU OLIVIAAlthough the origin of nursing predate the mid -19th century, the history of professional nursing traditionally begins with Florence Nightingale. Nightingale, the well-educated daughter of wealthy British parents, defied social conventions and decided to become a nurse. The history of nursing first started to become more continuous and defined with Christianity when Christians cared for the sick, fed the hungry and buried the dead. Therefore, it was said that the history of nursing is tied to the church. When it became apparent that love and nurturing alone were not enough to cure disease, the need for a more educated frame work for nurses began to form. As a result of this, Florence nightingale in 1860, fulfilled her dream concerning nursing by establishing the Nightingale Training School for Nurses. It was the first formal, fully organized training program for nurses. In this 21st century, many nursing schools have been established to help build on the previous skills and experiences that were acquired through long years of housekeeping, assisting in child bearing and care of formal sick relatives. The Patient/Family Care Study is a detailed written report of nursing care rendered to an individual and family within a specific period of time. In the model of contemporary nursing care, every patient will have a nurse who is the primary decision maker when it comes to their care plan during their hospital stay and if it was readmission. It explores nursing rendered to patients from time of encounter to the termination of nurse-patient relationship. It gives an in depth description and explanation of how a patient’s response to a specified disease condition is diagnosed and given intervention. The Patient/Family Care Study involves a record of nursing care, identifying the problems of a nursing patient and how they are dealt with by the nurse in the course of finding solution to the problems. It provides a systematic way of collecting data, analysing information, and reporting the results of nursing care. Nursing care when rendered properly helps the patient to meet his physical, psychological, social and spiritual needs. Currently, patients are nursed using the nursing process. The nursing process is goal-oriented method of caring that provides a framework to nursing care. The nurse ensures that the patient is assessed, diagnosed, and receives continuity of care across appropriate healthcare providers and departments. It involves six major steps which are assessment (what data is collected), diagnosis (what is the problem), outcome identification (objective/outcome criteria), planning (how to manage the problem), implementation (putting plan into action), and evaluation (did the plan work). This Patient/Family Care Study is based on the concept of holistic care, taking into account all factors affecting the health of the individual. It therefore involves the interaction between the patient, his family, the community in which he stays and the health team. It is done using the nursing process approach. Evidence-based practice is a conscientious, problem solving approach to clinical practice that incorporates the best evidence from well-designed studies, patient values and preferences, and a clinician's expertise in making decisions about a patient care. This care study is carried out in partial fulfilment of the requirement for the award of professional license by the Nursing & Midwifery Council of Ghana. It encourages learning by doing, development of analytical and decision-making skills as well as reporting skills. Based on the nursing process, the students become familiar with the use of the nursing process as a basis for practice thereby encouraging evidenced based nursing care. For the purpose of confidentiality and anonymity, initials of the characters are used instead of their real names.

ItemA Patient/ Family Care Study on Gastritis(Holy Family NMTC Library, Berekum, 2022-08-01) Adu-Adjei Mispa FranciscaPreviously, nursing was just caring for the sick on the sick bed. The nursing profession began to change rapidly under the influence of Florence Nightingale. Nursing has changed from caring for the sick to include taking of medical history and conducting physical examination which was previously the duty of the medical doctor. According to Virginia Henderson, nursing is the process of assisting the individual either sick or well in the performance of those activities which contribute to health or peaceful death that he would have performed unaided if he had the necessary strength, will or knowledge and to do this in such a way as to help him gain independence and rapidly as possible. Due to modernization, nursing has been changed to a holistic and individual nursing care of a client by means of new techniques employed in the profession. To provide holistic and efficient nursing care to patient and family, the student nurse employs knowledge and skills in all areas of discipline, such as; psychology, sociology, surgery, pharmacology, public health and medicine to meet the needs of the client, family and community as a whole. Patient/Family care study is a written script on individualized nursing care rendered to a patient in relation to his disease condition at a specific period of time. The care is based on the theoretical and practical experienced acquired by the student nurse through the three-year training. The study forms part of the assessment of the student nurse by the Nursing and Midwifery Council of Ghana for the award of Professional Diploma Certificate. The patient’s care was carried out making use of the scientific approach to nursing care which is the nursing process. The study helps the student to gain knowledge in all areas of medical science to care for clients as individuals. The patient and family care study starts from the day of admission to the time of discharge and continue in the community to ensure optimum health through home visit. For the purpose of confidentiality, the name of my patient and family relatives were stated using initials throughout the care study.

ItemPatient/Family Care Study on Hypertension(Holy Family NMTC Library, Berekum, 2022-08-01) SAASOR HAPPYCenturies ago, the nursing profession was just caring for the sick. From the day the profession started, it has undergone many changes throughout the years. Starting from the patient centered approach to a more complicated form of care which includes: the family and the community as a whole. Nursing became a profession when Florence Nightingale, in the nineteenth century provided and set a pattern which has become the basis of educating nurses today. Nursing care has moved from the physical care of patients to a more complex approach which includes the psychological, spiritual and intellectual needs as well as that of their family as a whole (Nursing process). To achieve ultimate goal, the nursing process requires the cooperation of the family, the patient and the community. Also, education is given to help prevent the disease and to help them to know their right. pertaining to their care. The patient and family care study entails rendering holistic care to the patient and family starting from the day of admission till discharge. Individualized nursing care is the area of interest in the study. The nursing process is a deliberate problem-solving approach for meeting a person’s health care and nursing needs. It consists of a sequence of steps in the following order; assessment, diagnosis, objective/outcome criteria, planning, implementation, and evaluation. The patient/family care study also offers the student nurse the opportunity to put into practice the knowledge acquired at school in giving effective nursing care to client with reference to the client’s condition. In addition to the above, the patient/family care study enables the student to acquire more knowledge about the causes, signs and symptoms, diagnosis and treatment given to patients with specific conditions using the nursing process. The confidentiality of the patient/family was ensured by the use of patient/family members initials instead of full names. The patient/family care study is a requirement for the award of the license to practice as a Registered General Nurse by the Nursing and Midwifery Council of Ghana. The comprehensive care rendered was made possible by the employment of skills and knowledge in such disciplines as psychology, anatomy and physiology of the human system, public health nursing, medical nursing, surgical nursing, pharmacology and nutrition and dietetics

ItemPatient/Family Centered Nursing Care Study on Gastroenteritis(Holy Family NMTC Library, Berekum, 2022-08-01) AGYAPOMAA CYNTHIAThe Nursing profession today has gone through evolutions to reach where it is now today. Nursing was not taught in the past, until the 1800's where it became an organized practice. In 1854, Nightingale had the opportunity to test her beliefs during the Britain’s Crimean War. The British Government asked Nightingale to take a small group of nurses to the military hospital at Scutari. Nightingale within days of arrival reorganized the hospital, scrubbing the walls for sanitation, windows opened for ventilation, nourishing food was prepared and served with medications and treatments administered efficiently. Death rates plummeted in weeks, with the graceful public knowing of the work done by the ‘’Lady with the Lamp’, who made night rounds giving comfort to the sick and wounded. Nightingale pushed for reform of hospital sanitation methods and invented methods of graphing statistical data. Nightingale’s work led to drastic changes in army medical care, the establishment of army Medical School and medical records, and ignited the growth of nursing as an organized profession. For these contributions, Nightingale is widely accepted as the founder of nursing. The nursing profession gained a lot of recognition and support from civilians during this time, as they realized the essential asset to medical care that nurses truly are. In the early 1900's, nursing education was received primarily from hospitals rather than colleges or universities. The training of nurses in diploma program, licensing of nurses, specialization of hospitals, diagnosis, and advanced degree programs, scientific and technological development are been offered today. This therefore has effectively improved upon the quality of health care delivered by the use of the nursing process. The nursing process is a series of organized steps designed by nurses to ensure a planned and quality of patient care. This helps to assess patient needs and to also derive various means of intervening. It provides an organized framework for the practice of nursing and the knowledge, judgments, and actions that nurses bring to patient care. The process requires effective thinking, activity and affective skills and abilities of the nurse. The patient and family care study is a comprehensive and extensive description of the entire nursing care rendered to a patient and the family during and after hospitalization period. This is to meet the physical, emotional, psychological, spiritual and social economic needs of the patient and her family within a specific period of time. This care study, forms part of the assessment of the final year student in the awarding of License to practice as a professional Registered General Nurse. The patient and family care study helps widen the scope of knowledge of the student nurse, helping the student nurse to put his theoretical knowledge and skills acquired through training together to give a comprehensive nursing care to patient and family. The patient and family care study also enhances the interpersonal relationship of the student nurse as he constantly communicates with the patient, relatives, friends and other health care team in the various units of the hospital in order to provide a comprehensive care for the patient. For the purpose of confidentiality and security reasons, my patient’s identity will be disclosed; A.H will be used to represent my patient’s name throughout the script. I also admonish students and readers to always try to read prefaces which are a way of expanding our scope of knowledge and understanding.

ItemA Patient and Family Care Study on Uncomplicated Gastritis(Holy Family NMTC Library, Berekum, 2022-08-01) Asare PepertualHealth care is undergoing drastic changes globally, posing challenges to all categories of health health care. In line with these challenges, Nursing has to be considered alongside the work of other health care professionals. Undoubtedly however, a range of nursing services is required to respond adequately to the health needs of all patients. Rather than treating the patient as a disease entity, the nursing process enable the nurse to give a holistic care to patient with reference to their physical, psychological, physiological, social and spiritual needs of the patient. Today’s Nurse Trainee should be equipped with requisite knowledge, skills and attitude for the new challenges of their role in society. The patient/family centered care study method is an opportunity for the Nurse Trainee to translate his theories of Nursing, especially the Nursing process concept into reality, in the clinical setting. The patient/family care study is a requirement by the Nursing and Midwifery Council of Ghana for every final year nursing student to undertake in. It is a script prepared by a final year student nurse on a patient. The study is to render a scientific based holistic care on the patient till his full recovery using the nursing process. The care study offers the best chance for a final year nursing student to show his/her competency in rendering care to a patient till his full recovery. It also gives the student nurse the chance to interact and co-ordinate with other members of the health team and for the promotion of optimal health to individuals and communities at large. It also helps to raise the standard of nursing education as it ensures a comprehensive and individualized care given to patient and community as a whole. I conducted this patient and family care study because as a final year student, it is a requirement by the nursing and midwifery council of Ghana to carry out such a study in partial fulfilment for the award leniency to parties as a registered general nurse in Ghana. For confidentiality to prevail, initials of the patient/family are used instead of their identity.

Many people believed that Nursing started with Florence Nightingale, however nursing itself dates back to the beginning of motherhood when nurses were traditionally female. The history of nursing has its The history of nursing first started to become more continuous and defined with Christianity when Christians cared for the sick, fed the hungry and buried the dead. ItemA Patient/Family Care Study on Gastritis(Holy Family NMTC Library, Berekum, 2022-08-01) TUAH ESTHERAlthough the origins of nursing predate the mid-19th century, the history of professional nursing traditionally begins with Florence Nightingale. Nightingale, the well-educated daughter of wealthy British parents, defied social conventions and decided to become a nurse. The nursing of strangers, either in hospitals or in their homes, was not then seen as a respectable career for well-bred ladies, who, if they wished to nurse, were expected to do so only for sick family and intimate friends. In a radical departure from these views, Nightingale believed that well-educated women, using scientific principles and informed education about healthy lifestyles, could dramatically improve the care of sick patients. Moreover, she believed that nursing provided an ideal independent calling full of intellectual and social freedom for women, who at that time had few other career options. Nursing care has evolved from just caring for the sick and the dying, to an era of assisting people who seek health guidance and counselling, as well as promoting the health of individuals, their families and the entire community. There has also been an extension of care to the sick person’s family and community, at large, in all aspects of health care. Nursing in the past four decades have brought emphasis on nursing research and the use of scientific data at the bedside. Nursing care has broadened from care of the sick to care of the people both in sickness and health and also extend to the patient’s family and community at large in all aspects regardless of the background. The Patient/ family care study is a detailed account of nursing care rendered to the Patient and family to meet their needs. The study is designed to give a comprehensive nursing care to both patient and family from the time of admission till when patient is finally discharged to go home, as well as follow-ups or home visits for continuity of care. The study provides a systematic way of collecting data, analysing information, and reporting the results of nursing care. This Patient/Family care study is based on holistic care, taking into account all factors impinging on the health of the patient. The Patient/Family care study forms an integral part of the curriculum for educating nursing students hence a prerequisite for completing the nursing course and also a partial fulfillment of the requirement for the award of professional license by the Nursing and Midwifery Council of Ghana. Using the nursing process in caring for a patient, emphasis is based on health promotion, maintenance and restoration or enhancing a peaceful death depending on the patient’s condition. The nursing process is a series of organized steps designed for nurses to provide excellent care. This involves five phases, including assessing, diagnosing, planning, implementing and evaluating. The purpose of nursing process is that, it offers a framework for thinking through problems and provides some organization to nurse’s critical thinking skills. It’s important to point out that this process is flexible and not rigid. It is a tool to use in nursing care, but one that should allow for creativity and thinking outside the box. The study is carried out to enable the student nurse put into practice the knowledge and skills acquired from the three-year training period in school to ascertain how best the theoretical knowledge would be used to nurse patients who will come under his or her care in the near future. Initials were used instead of the patient’s full name to maintain confidentiality. The study serves as a reference paper for other student nurses and qualified health personnel who may be interested in its content.

ItemA Patient/Family Care Study on Acute Pyelonephritis(Holy Family NMTC Library, Berekum, 2022-08-01) ASUM FREDANursing was “untaught” and instinctive. It was performed out of compassion for others, out of the wish to help others. Nursing was a function that belonged to women. It was viewed as a natural nurturing job for women. Nursing emerged as a profession in the mid-19th century. Historian’s credit Florence Nightingale, a well-educated woman from Britain, as the founder of modern nursing. Nightingale challenged social norms – and her wealthy parents – by becoming a nurse. At the time, the public objected to the idea of women nursing strangers. But Nightingale saw nursing as an extraordinary opportunity for females. She believed they could use their education and scientific knowledge to improve patient care while gaining personal independence. In 1854, during the Crimean War, the British government requested Nightingale’s aid at a military hospital in Turkey. Within weeks of her small team arriving, the mortality rate of British soldiers fell dramatically. Nightingale’s accomplishments impressed the public and ultimately helped convince the Western world of the dignity and value of educated nurses. One prominent change in the evolution of the nursing profession is formalized education. The first training programs opened at hospitals in the late-19th century. Student nurses received clinical instruction in exchange for providing care to patients. During this period of training, nurses helped hospitals make tremendous improvements in safety and quality, and humanized medical care. By the second half of the 20th century, patient needs became more complex and hospitals required skilled nurses to manage them. However, it ought to signify the proper use of fresh air, light, warmth, cleanliness, quietness, the proper selection of diet and so on (Nightingale, 1820-1910). It also enables the student nurse to acquire knowledge and skills on interpersonal relationship and communication which are essential ingredients for the provision of competent, holistic, professionally based and quality care to the patient and family.
